Whether an individual requires 24-hour assistance or periodic support, we design services based on personal goals, preferences, and needs, offering personalized in-home and community support to meet individualized goals.
Key Areas of Support:
- Autonomy & Decision-Making – Encouraging self-advocacy and informed choice

- Health & Well-Being – Medical appointments, medication management, and support with activities of daily living
- Emergency Preparedness – Training on personal safety, health management, and emergency preparedness
- Community Engagement & Safety – Social integration, daily living skills, and skills discovery
- Personal Growth & Independence – Assistance in creating a stable and fulfilling lifestyle
Duration of services is dependent on individual needs, whether short-term or long-term.
