Self-Determination Program

Expanding Access Through Self-Determination

Connections For Life continues to evolve and expand our programs to meet the changing needs of the community. We believe that access to information and resources is a vital part of empowering individuals with disabilities to lead self-directed lives. In alignment with this commitment, CFL has extended our program designs to include individuals participating in the Self-Determination Program (SDP).

Established in October 2013, the Self-Determination Program was signed into law by Governor Edmund G. Brown Jr. as an alternative service model, providing individuals with the ability to select and direct their own services through person-centered planning. As outlined in Welfare and Institutions Code, Section 4685.8, this voluntary program allows participants to create a customized mix of services and supports designed to help them achieve personally defined goals in inclusive community settings.

Through CFL’s longstanding Supported Living Services (SLS) and Independent Living Services (ILS) programs, we offer a structured foundation that aligns with the principles of self-determination. Our team works closely with individuals to navigate available resources, coordinate services, and support their vision for independent living, health, and overall well-being. We are dedicated to ensuring that every person has the opportunity to access services that promote choice, inclusion, and meaningful engagement in their community.
